If you are part of a multi-person household, you only need to input one name in the first field. The second field can be either an email address or a mobile phone number, not both. This will be used for verification purposes when electronically signing. When the agreement is completed, you will get an executed copy sent to your email or via text to your phone dependent upon the contact method provided.
